Purpose: to identify problems of accepting Indonesian nurses and care givers to work in Japan in the framework of Economic Partnership Agreement between Japan and Indonesia and to propose possible solutions. Subjects and methods: Eighty-six Japanese staff working in 9 facilities of 'Tokubetsu Yougo Roujin home (Long-term care Nursing Home)' and 1 hospital located in Kansai area and 69 Indonesian health workers who will work in Japanese health facilities participated. Data from a set of questionnaires given to participants was collected between August and December 2008. Interviews were conducted for 48 managers and 38 staff from 9 Japanese health facilities. Permission for this study was obtained from managers of the facilities and each participant through their informed consent. Results: For Japanese participants 40 (47%) agree/accept to work with Indonesian workers while 46 (53%) disagree/reject. Fifty-five percents of the accepting group and 41% of the rejecting group responded that the examination for professional knowledge and the examination for Japanese writing and knowledge ability should be separated. Both groups responded that passing the National nursing or care giver board examination is no indication of Japanese writing and knowledge ability. Among Indonesian participants, the main reasons given for working in Japan were career development (83%), higher salary (59%), and further education (57%). Main concerns of daily living in Japan were language difference (93%), culture/ religion difference (78%), and foods and lifestyle difference (75%). Main supports needed by Indonesian workers were, to master Japanese language (37%) and mental, moral, motivation and psychological support (31%) . Conclusion: Separation between knowledge and practical test in the National examination for care giver is desirable. The National examination held in Japanese is no indication of foreign workers' knowledge and skills. It is difficult for foreign workers to pass the National examination within a set period of time consequently special consideration is required for them.
