慢性閉塞性肺疾患 (COPD) 患者における気管支拡張剤吸入の肺機能及び運動時肺循環動態に及ぼす効果を検討するため, 臨床的に安定したCOPD患者12例を対象にβ_2刺激剤であるプロカテロール (P) 及び抗コリン剤であるイプラトロピウムブロマイド (I) の吸入併用時の肺機能測定を行った。P, Iいずれの吸入でも有意の1秒量 (FEV_1) の改善を示した。併用効果については, 1の60μg吸入によりFEV_lは最大となった (0.84±0.32 [平均±SD]→1.14±0.39L)が, Pを20μg追加吸入しても有意な増加はなく併用効果を認めなかった。対象12例のうち informed consent の得られた6例に右心カテーテル検査を施行し, 仰臥位エルゴメーターによる運動負荷での肺循環動態をPの20μg吸入前後で測定した。安静時の肺循環動態はP吸入前後で変化を認めなかった。P吸入により運動後の平均肺動脈圧 (mPAP), 右室一回拍出仕事係数 (RVSWI) の上昇度が抑制され[mPAP : 43.7±4.4→34.8±3.9mmHg (p<0.05)]。以上, COPD患者における気管支拡張剤の吸入は肺機能を改善させ, 運動時肺高血圧の程度を軽減し右心負荷を減弱させた。これは気管支拡張剤吸入の気管支拡張効果による換気効率改善とそれによる auto-PEEP 改善の寄与が推測された。
For the patients with COPD, quality of life (QOL) is severely impaired by irreversible airflow limitation and the prognosis is intimately associated with the severity of airflow limitation and pulmonary hypertension which are induced by alveolar destruction provoked by tobacco smoking. Two kinds of inhaled bronchodilator, anticholinergic agents (ipratropium bromide ; IB) and β2 receptor stimulants (procaterol ; PR), were examined for the evaluation of the beneficial effects of inhaled bronchodilators on pulmonary function in 12 stable patients patients with COPD. In addition, pulmonary hemodynamics during exercise before and after inhalation of 20μg PR was examined in the six patients of them through the right heart catheterization with supine cycle ergometer. Both bronchodilators showed signficant bronchodilating effects. The time to reach the peak of the improved forced expiratory volume in one second (FEV 1) was superior PR to IB, although the maximal bronchodilating effects were superior IB to PR. Additional inhalation of 40μg IB after initial 40μg PR showed significant increase of FEV1 from 1.12±0.42 to 1.18±0.44L, but additional inhalation of 20μg PR after initial inhalation of 60μ g IB did not. The results of the effects of inhaled bronchodilator on the pulmonary hemodynamics during exercise showed that the mean pulmonary artery pressure and the right ventricular stroke work index were significantly decreased (43.7±4.4 to 34.8±3.9mmHg, p<0.05, and 32.2±8.5 to 24.7±5.0g・m/m^2, p<0.05, respectively), and tidal volume during exercise was increased from 970±156 to 1152±235mL (p<0.05). We conclude that bronchodilators, especially anticholinergic bronchodilator, have beneficial effects on the pulmonary function and reduce the severity of pulmonary hypertension and the right ventricular working capacity in patients with COPD. Inhaled bronchodilator to the disabled patients with COPD may improve QOL and the prognosis.
